All children must be fully registered before they may attend the center. The annual registration fee is $35 per family. Registration may take place in person at the time of your first visit. You may also download the registration forms online and bring the completed forms with you at the time of your first visit.

To complete your registration, please download, print, and fill out the forms below.

Children OVER 5 years of age:
Please complete Form 1 ONLY.

Children UNDER 5 years of age:
Please complete BOTH Form 1 AND Form 2.
NOTE: All that is required at the time of your first visit is Form 1.
Form 2 must be submitted to us within 30 days of your child’s first visit.
